Open Safari (not Chrome) on the iPhone or iPad and go to joinhomebaseapp.com/homebase/dashboard/

In the menu that slides up, scroll down and tap "Add to Home Screen" — it has a plus icon in a box

Tap Add in the top right corner. The app icon will appear on the home screen.
Step-by-step

Open Chrome on the Android device and go to joinhomebaseapp.com/homebase/dashboard/

Look for a banner at the bottom of the screen that says "Add Home Base to Home screen" — tap Install if it appears

If you don't see the banner, tap the three dots ⋮ in the top-right corner of Chrome
